Cigna credentialing for therapists: what to expect and what you'll earn
Cigna is one of the four major commercial insurance networks for behavioral health, operating its behavioral health benefits through a subsidiary called Evernorth. If you receive a credentialing letter from Evernorth Behavioral Health, that is Cigna's behavioral health network. Here's how credentialing works, what rates look like, and how to decide if it's right for your practice.

Credentialing with Cigna / Evernorth?
paneled.ai handles Cigna and all four major networks in a single intake — CAQH, NPI verification, application filing, and follow-up.
Start your Cigna applicationCigna and Evernorth: understanding the relationship
Cigna rebranded its behavioral health subsidiary as Evernorth in 2020. As of 2024, behavioral health credentialing, claims, and provider relations for Cigna members all go through Evernorth Behavioral Health. This affects every aspect of the credentialing relationship — from the name on correspondence to the phone number you call when following up on an application.
If you receive correspondence addressed from "Evernorth Behavioral Health" or "Evernorth Health Services" regarding credentialing, that is Cigna. There is no separate credentialing entity. Evernorth IS Cigna's behavioral health network. The two brands refer to the same insurance relationship — Cigna is the parent commercial insurer, Evernorth is the behavioral health subsidiary that handles your provider contract.
This is a common source of confusion. Providers sometimes apply to Cigna directly and don't recognize Evernorth correspondence as related, or check their credentialing status under "Cigna" only and miss updates filed under "Evernorth." The practical implication: when checking your credentialing status or responding to information requests, check both "Cigna" and "Evernorth" in any portal or documentation. Similarly, when authorizing payers in CAQH, both entries may appear and both may need to be authorized.
Cigna/Evernorth manages behavioral health for Cigna commercial members, Cigna Medicare Advantage plans with behavioral health benefits, and many employer-sponsored plans that use Cigna as their benefits administrator.
Cigna reimbursement rates for therapists
These rates are from CMS Transparency in Coverage data — federally mandated filings published by Cigna. They represent the P20–P80 range of observed rates. Your specific rate will depend on your state and contract.
| CPT code | Session type | Rate range |
|---|---|---|
| 90837 | 60-min therapymost common | $68–$151 |
| 90834 | 45-min therapy | $63–$105 |
| 90791 | Initial evaluation | $70–$161 |
| 90847 | Family therapy | $67–$118 |
| 90785 | Interactive add-on | $3–$13 |
Ranges are P20–P80 from CMS Transparency in Coverage data. Rates vary by state, locality, and contract negotiation. See your exact contracted rate on upgrate.ai
Cigna's rate distribution for CPT 90837 shows a wider spread than some other major payers, which reflects significant variation by state and market. The lower P20 figure can mean lower floors in some markets — worth checking your state-specific data before applying, especially if you practice in a market where Cigna doesn't have a strong commercial presence. The upper end is competitive with other networks.
For state-specific rate comparisons, upgrate.ai uses the same CMS data with state and ZIP code filtering, which lets you see what Cigna actually pays providers in your specific area before you commit to the application process.

What Cigna requires for behavioral health credentialing
Cigna/Evernorth follows a standard credentialing process built around CAQH and primary source verification. Before applying, make sure all of the following are in place:
- Active NPI with the correct behavioral health taxonomy code — a mismatched taxonomy can stall the application at the primary source verification stage (LCSW: 1041C0700X · LMFT: 106H00000X · LPC: 101YP2500X · LMHC: 101YM0800X · PsyD/PhD: 103T00000X)
- Complete CAQH ProView profile, fully attested within the last 120 days — Cigna/Evernorth uses CAQH for primary source verification and will not process an application without it
- Active state license(s) with no disciplinary actions
- Active malpractice insurance with no gaps in coverage — gaps trigger additional review and can delay the credentialing committee decision
- Work history and references submitted through CAQH — make sure your work history is complete for the past five years with no unexplained gaps
- Evernorth must be specifically authorized in your CAQH payer authorization section — look for both "Cigna" and "Evernorth" in the payer list and authorize both entries
On that last point: when authorizing payers in CAQH, look for both "Cigna" and "Evernorth" — the entry may appear under either name depending on when your CAQH account was created. If only one is authorized, Evernorth may not be able to pull your profile during the credentialing review.
CAQH and NPI: prerequisites before applying to Cigna
If you don't have a CAQH ProView profile yet, setting one up is the first step before any commercial credentialing application. CAQH is the universal credentialing data repository used by all four major networks — your profile, once complete, supports concurrent applications to Cigna, Aetna, Anthem, and UHC without duplicating data entry. See our step-by-step CAQH setup guide.
CAQH ProView requires re-attestation every 120 days. Cigna's credentialing review takes 60–90 days — if your CAQH profile lapses during that window, your application stalls. Set a calendar reminder when you submit.
You also need an active NPI (National Provider Identifier) before applying. Your NPI must be registered with the correct behavioral health taxonomy code — if it's missing or incorrect, Cigna/Evernorth's credentialing system may flag the application during the primary source verification step. This is one of the most common preventable delays in the credentialing process. See our guide to getting and maintaining your NPI.
How long Cigna credentialing takes
Cigna behavioral health credentialing typically takes 60–90 days with a complete application. Among the four major commercial networks, Cigna is generally on the faster end — similar to Aetna and somewhat faster than UHC or Anthem in many markets. The 60-day outcome is achievable when your CAQH profile is complete and attested, your NPI taxonomy is correct, and your malpractice certificate matches the dates in your CAQH profile.
The main variable is your CAQH profile. If it's complete and up-to-date when you apply, the process moves faster because Evernorth's credentialing team can pull verified data without sending information requests. Credentialing committee cycles at Cigna/Evernorth typically run monthly or quarterly depending on your region, so a returned application — or one that comes in just after a cycle closes — adds approximately 30 days to the timeline. Submitting a complete application at the beginning of the month improves your odds of hitting the next committee date.
Is Cigna currently accepting new behavioral health providers?
Cigna/Evernorth panel status varies significantly by state and region and can change quarter to quarter. Cigna is known to manage panel capacity regionally — a panel that's open in your county today may close in six months as the network reaches capacity targets. This makes checking status an important step before investing time in the application.
To check status, contact Evernorth's provider relations team directly. Note that the behavioral health contact is not the same as Cigna's general provider line — you need the Evernorth Behavioral Health provider relations number, which routes to the team that actually manages your credentialing. Ask specifically about your license type (LCSW, LMFT, LPC, etc.), your practice ZIP code, and the specialties you intend to list on the application. The answer can differ for each of these variables.
If the panel is currently closed in your area, ask to be waitlisted and whether any sub-specialty designation or underserved-area status would qualify you for an expedited panel opening. Cigna/Evernorth, like other major payers, will sometimes open a closed panel to providers who can fill a specific specialty gap or serve a geographic area with documented access issues. It's worth asking — the answer costs nothing and occasionally changes the outcome.
Not sure if Cigna is open in your area?
paneled.ai checks Cigna / Evernorth panel status for your license type and state before you apply — and adds you to the waitlist if the panel is currently closed.
Check Cigna panel statusIs Cigna worth credentialing with for your practice?
Cigna's rate variability by state means the answer depends heavily on where you practice. In markets where Cigna's rates are at the upper end of the spread, it's a straightforward inclusion. In markets where the P20 floor is notably lower than other payers, it's worth running the state-level numbers before committing. The CMS Transparency in Coverage data — filtered to your state — gives you the most accurate picture available before you have a contract in hand.
The Evernorth brand situation is worth keeping in mind throughout the process. You want to make sure you're engaging the right team from the start — use the Evernorth Behavioral Health provider relations contact, not the general Cigna provider line — and that you're checking both "Cigna" and "Evernorth" in every portal and document. That clarity upfront prevents the most common administrative friction in the Cigna credentialing process.
For practices that want four-network coverage and maximum reach, Cigna is a natural inclusion in the bundle. When you apply to all four networks concurrently, the incremental effort for the fourth payer is minimal — your CAQH profile already supports the application, and the admin overlap is high. The question of whether Cigna makes sense in isolation is a harder calculation; in the context of a concurrent four-network application, the incremental cost of including it is low relative to the coverage it adds.
One honest note: credentialing doesn't guarantee panel acceptance. Panel decisions belong to Cigna/Evernorth, and a complete application to an open panel is the precondition — not the guarantee — of getting in-network. We never promise panel acceptance, and you should be skeptical of anyone who does.
